I finally found the time to read ” In Stitches” a memoir of Plastic Surgeon, Anthony Youn, M.D. I am not in the habit of writing book reviews, however I have been looking forward to reading this book since it was released. Dr. Youn is recognized as a top plastic surgeon and has been seen on shows like Rachel Ray and Dr. 90210. His blog “Celebrity Cosmetic Surgery” has been hugely popular, and now he shares his personal journey to becoming a plastic surgeon in his book “In Stitches”. This is a fun, easy read, a perfect book for a relaxing summer day.

In Stitches gives a humorous inside look of the life of Anthony Youn, a boy that grew up feeling like a “nerd” who didn’t fit in, and his journey to becoming a successful Plastic Surgeon. As a teen he was insecure about his over sized jaw and had major plastic surgery to correct it. He hoped that after the surgery he would have better luck with girls, but things don’t always work out as planned. Dr. Youn shares the personal struggles he had talking to women and trying to get a date while in medical school. Honestly, I was beginning to wonder if he was EVER going to get lucky. I actually had to flip back to the photo of Dr. Youn on the inside flap several times, to see if it offered any clues to his dating challenge. I think it all comes down to confidence.

Dr. Youn said he never had an “ah-ha” moment when he instantly knew that he was going to become a plastic surgeon. Maybe he didn’t have an “ah-ha” moment, but I suspect that a night he spent in pediatrics was a major influence. It is often overlooked, that plastic surgery has it’s roots in reconstruction, we are reminded of this when an infant is brought into the emergency room in need of immediate plastic surgery to reconstruct his face.
You will have to read the book to find out exactly what happens. (Warning, this chapter is a tear jerker)

I have spent my career working with doctors, in the field of Medical Aesthetics and Plastic Surgery, so I have some idea how challenging medical school is and plastic surgery happens to be one of the most competitive fields in medicine. Youn was one of 250 qualified candidates competing for a residency in plastic surgery, with only 60 openings. After completing applications for 35 plastic surgery residency programs and 20 general surgery programs (as backups), he receives 15 interview invitations for general surgery and 8 for plastic surgery. Now begins the exhausting process of traveling around the country for residency interviews. These interviews are a bit unusual and a little comical. Finally, Match Day, the day when future doctors are matched with their residency via a process that is compared to E-Harmony.

I know that I am supposed to be the “skin guru”, but occasionally I learn something from my clients. Years ago, one of my clients went to Israel. Before she left, she promised she was going to bring me back a little gift. Yay…I love gifts! ( I am blessed with the most amazing clients.) She returned with AHAVA mineral hand cream. It’s been all over the news, I am referring to the Esthetician who gave her eight year old daughter injections of Botox to treat “wrinkles”. She was motivated to do this to help give her daughter an advantage in pageants. As a mother and an Esthetician myself, I wanted to share my thoughts on this crazy story. It has already been said that she is sending the wrong message to her daughter. Lets face it, that message began with putting an eight year old in a beauty pageant.

Why Does An Eight Year Old Want Botox?

There has been discussion about why an eight year old would ask for Botox in the first place. As an Esthetician I can tell you that at that same age my son knew about many of the things we offer in the beauty industry, just from hearing me talk about it. I even remember him telling me that his teacher needed a blue peel, but for the most part he had no interest in esthetic treatments. I am sure that girls would naturally have a little more interest in esthetic and cosmetic treatments than boys, but certainly not anti-aging treatments. Just to state the obvious… children do not have wrinkles!

Mommy is an Esthetician????

The mother of the little girl is an Esthetician and claims it was her daughter that asked for Botox. As a mother and an Esthetician, I do not see how that justifies her actually injecting her daughter with Botox. My son once asked me to remove his cow lick with laser hair removal. I obviously refused and explained to him that although it would permanently rid him of his cow-lick, he would be left with a bald spot on the back of his head. He said, he would rather have a bald spot than a cow-lick. I promised him that he would feel differently later, and then changed the discussion to the pain involved. Now that he is a teenager, I know he is glad I didn’t honor his request. My first point is that children do not know what is best for them, thats why they have parents. My second point is, Estheticians have a professional responsibility, to educate clients about the benefits and risks of a treatment.

The Worst Esthetician Ever!!!

The little girl said she didn’t like wrinkles. Okay… that is fair, I don’t exactly like them either. I am going to be completely hypothetical for a moment, because I am obviously not in the business of treating or consulting children for cosmetic treatments. If I was consulting with this little girl, I would point out that she doesn’t have wrinkles and I would explain to her that children have a lot of collagen and elastin and that their skin cells turn over very fast. I would also explain that the facial muscles only become strong enough to create lines and wrinkles after many years and she could reconsider Botox in about 20 years. I would teach her that the best way to prevent wrinkles is to use a good sunblock every day. Other things she should do, is eat a healthy diet with a lot of fruits and vegetables, drink plenty of water, get enough sleep and exercise and never start smoking. I might even teach her how to properly cleanse her face, but thats about it. It is irresponsible as a skin professional to provide skin treatments that are unnecessary to anyone. I have to assume that either this esthetician doesn’t understand how the skin functions or has poor ethical and professional standards (or both). She is also working outside the scope of an Esthetic License, the only way she can inject Botox is under the direct supervision of a doctor, in which case the doctor can be held responsible for her actions. She stated that she does the injections her self, and purchases the Botox online. FYI- the internet is not a trusted source. After careful consideration, I am awarding this mom with the title of ” Worst Esthetician Ever”.

I am annoyed that this ridiculous story gives a bad reputation to Estheticians and Botox. The truth is there are “bad” Estheticians, which is why it is so important to find an Esthetician that excels in the field. I have to advocate for Botox, by saying that it is only intended for cosmetic use in consenting adults and in children to treat specifically indicated medical conditions, but not for cosmetic purpose. Botox is only sold to approved doctors, the woman in this case claims to have purchased it on line. As I have said many times before, just because you can buy something on the internet doesn’t mean you should.

M∙A∙C Spring Colour 2011 is the feminine force that saves the day! No nemesis can match her super-sized Mineralize Skinfinish, Eye Shadows, Lipsticks and Powder Blush for this transformation!

When my BFF, sent me a photo from her phone of the M.A.C. counter decked out and covered with Wonder Woman products, I was totally elated. Anyone that knows me, should know I am a huge Wonder Woman fan! Unfortunately, I wasn’t able to make it to the M.A.C. cosmetic counter for a couple weeks, and when I finely went to the M.A.C. counter at Paradise Valley Mall, I was disappointed to learn that they had sold out of almost the entire collection. Apparently, this limited edition collection was very popular. The packaging is nostalgic of the Wonder Woman from the comic book series and the cosmetic M. A. C. has even put together a series of Wonder Woman looks: Classic Wonder Woman, Defiance, Golden Lariat, Deep Truth, Mighty Aphrodite and Marine Ultra.

I was so excited and surprised, when my Aunt showed up at my house with the Wonder Woman makeup case and the cosmetic products that they still had left in the collection. I have also been getting text and Facebook messages from my friends about any of the Wonder Woman products they find. If you are looking for the Wonder Woman cosmetic collection, rumor has it that the
M. A. C. counter at the Arizona Biltmore still has some products available. I would like to send out a huge thank you to all my friends and family that have supported my Wonder Woman addiction.

My Personal Bra Fit Experience

I can now say that had have had the true bra experience. No woman should miss this experience. Sorry to say, but Victoria’s Secret does not even compare to the {intimacy} experiance. This is a “real” customized bra fitting and with the highest quality of bra’s and lingerie. It was obvious that my bra specialist has been through extensive training to ensure the best service possible. I was delighted to learn they offer free custom alterations!

I feel like a new girl with this awesome bra. I really loved the bra I had recently purchased from Victoria’s Secret, it was comfortable and cute, but the problem was that it wasn’t giving enough support. In my case the band size I was wearing was too big. I had no idea how involved a true bra fitting really should be. I learned why it is important to see a bra specialist and to wear the correct bra size. For example, if you don’t wear the correct bra size, (which many of us don’t) you could be causing strain and damage to your breast tissue which can contribute to sagging of the breasts.

I strongly recommend using a sunblock that contains Zinc Oxide. The challenge with Zinc Oxide is that it is very thick and white and not appealing for daily use. There are many products that offer a micronized Zinc, however on dark skin it still leaves a white cast on the skin. Skin Ceuticals created a sunblock that contains a transparent Titanium Dioxide and Zinc Oxide. rosacea treatments

Rosacea is primarily a facial vascular disorder in which blood vessels are functionally and structurally abnormal. Currently rosacea can’t be cured, however it can be effectively treated and controlled. Rosacea is a progressive skin disorder, that should be treated when symptoms first begin. Treatment should be aimed at treating the underlying vascular issues associated with rosacea. Vascular Lasers and IPL treatments are extremely effective at treating existing redness and broken blood vessels. In addition to avoiding rosacea triggers, topical products can be used to prevent flushing and further damage that can accelerate the progression of the disorder.

Laser and IPL Therapy

Both Laser and IPL therapy are very effective at treating telangiectasia (broken blood vessels), generalized redness and flushing. IPL therapy can also be enhanced with Levulan to get the added benefit of treating acne, rosacea papules and pustuals.

Laser

Lasers that are absorbed well by hemoglobin are used to treat vascular concerns. The laser energy is absorbed by the blood vessel and creates heat within the vessel which damages the vessel wall and permenatly closes that vessel. The treated vessels are then absorbed by the body. Although there isn’t a cure for rosacea, studies show that laser treatment can permantly remove damaged facial blood vessels and with repeated treatment blood vessels will not always be created to replace the old vessel. Facial blood vessels treated with lasers are sometimes replaced with connective tissue. This is good news, because this will permently reduce the number of blood vessels in the face, near to the level of vessels found in healthy skin.

Lasers have a single wavelength that work at a very specific depth. So any idividual laser has it’s limitations. KTP 532 nm and Pulsed Dye: 577 -585nm lasers are well absorbed by hemoglobin and are very effective at treating superfical blood vessels in the face, while Nd:YAG 1064nm lasers are also well absorbed by hemoglobin, they are effective at treating vessels that are located deeper in the skin.

Intese Pulsed Light (IPL)

IPL skin treatments are known by many names including Fotofacial, Photofacial, Photoderm and Photo rejuvenation. Intense Pulsed Light (IPL) differs from lasers because IPL uses a wide spectrum of wavelengths at the same time (for example: 500nm — 1200nm) that can target any chromophore in that range, while lasers have a single wavelength. You can think of the laser like a sniper that can precisely hit a single target with one shot, and IPL is like a grenade that hits several targets at once. IPL uses special cut-off filters that can be selected to target chromophors in a certain range within the skin. When treating rosacea or any vascular concern, the intended target is hemoglobin. The advantage IPL treatments have over laser treatments is that IPL can target vessels at different depths.

Combined Laser and IPL Therapy

Best results are achieved when you combine IPL and Laser treatments. IPL and Laser treatments can be done independently, however combining technologies at the same treatment time will provide a better result. This type of treatment is fairly aggressive and should only be done by someone that is very experienced in combining these technologies.

Oral and Topical Products

Topical Metronidazole Gel

Metronidazole is topical agent used to treat rosacea, available only by prescription. The primary action of Metronidazole is anti-inflamatory. Metronidazole is effective at reducing papules and pustules and has a fairly minimal effect at reducing inflammation. Unfortunately, it isn’t effective for treating broken blood vessels or flushing.

Antibiotics

Antibiotics have long been used to treat rosacea, however they work primarily through their anti-inflammatory properties rather than antibacterial. Antibiotics are effective at reducing papules, pustules, and inflammation. The limitation with antibiotics is that they are not effective at treating telangiectasia or flushing.

Antihistamines

Antihistamines are effective at reducing facial inflammation, swelling and burning associated with rosacea. Histamine contributes to swelling, itching and burning. Histamine has also been shown to play a role in inflammation and redness.

Sunblock with Zinc Oxide

UV rays can penetrate the skin and activate sensory nerves that release potent vasodilators, as well as creating heat in the skin causing facial flushing. Zinc oxide reflects both UVA and UVB rays, blocking out heat from the sun. Topical zinc oxide also has significant healing properties when applied to damaged skin and has been shown to decrease inflammation in both the epidermis and dermis.

Dimethicone

Dimethicone is a silicone based ingredient with moisturizing ability. Dimethicone is also a protective ingredient that has been shown to reduce flushing caused by skin irritants.

Antioxidants

Free radicals have been shown to cause structural damage to vascular walls and many free radicals are potent blood vessel dilators. Dietary and topical antioxidants help to block the damaging effects of free radicals.

Studies show that vitamin C can protect blood vessel walls from free radical damage and reverse existing vascular damage. People with rosacea should get plenty of vitamin-C in their diet, and may want to consider a vitamin-C supplement. In addition to being a great antioxidant, topical vitamin-C has anti-inflammatory qualities.
