rosacea
Rosacea is a vascular disorder that affects facial skin and eyes, causing facial redness, inflammation, broken blood vessels and red bumps known as inflammatory papules. Rosacea is a progressive disorder beginning with mild symptoms, and if left untreated can become severe. Rosacea treatment should begin as soon as possible to slow the progression, with an emphasis on treating the underlying vascular disorder. With continued treatment and avoidance of rosacea triggers, suffers can control their symptoms and slow or prevent further progression.

This post is intended for informational purposes only. Only a Doctor can diagnose rosacea, if you suspect you have rosacea you should consult with a physician, as facial flushing can be a sign of other systemic disorders. If you do have rosacea it is a good idea to begin a rosacea diary to track your specific triggers. Begin treatment as soon as possible to control your rosacea and do your best to avoid triggers.

Rosacea Stages: The progressing of symptoms

Pre-Rosacea

Pre-rosacea is characterized by frequent, temporary bouts of flushing and blushing. Facial redness disappears immediately after the trigger is over.

Mild Rosacea

Mild rosacea is characterized by flushing and redness that lasts for an abnormally long time after trigger is over (half hour or more). Broken blood vessels (telangiectasia) may be present and skin may be sensitive to products.

Moderate Rosacea

Moderate rosacea is characterized by facial redness that last days or even weeks. The nose and cheeks may have a semi-permanent redness, causing a sunburned appearance. Swelling, burning, broken blood vessels, and inflammatory papules may be present. Some people may experience eye symptoms such as irritation, dryness, crusting, sensitivity to light, swelling of the eyelids, and a feeling of a foreign body in the eye.

Severe Rosacea

Severe rosacea is characterized by severe flushing, inflammation, burning and swelling. Inflammatory papuals, pustules and nodules may develop. Rosacea that progresses to this point can be painful. In some cases rhinophyma may develop, this is a bulbous enlargement of the nose.

Function and Structure of normal blood vessels in facial skin

Function

Blood vessels deliver oxygen and nutrients to the skin, as well as remove waste generated from skin cells. It is important to understand that blood vessels help regulate internal body temperature. When the body internal tempeture is high, blood flow will increase through the vessel to release heat from at the skins surface.

Structure

Vessels are made up of several layers, each of which serves a specific purpose. When discussing rosacea, we are most concerned with the middle layer which is composed of vascular smooth muscle cells, and the inner layer which is composed of endothelial cells.

The smooth cells, or the muscular layer of blood vessels serve to control blood flow through dilation and constriction. When the muscular layer contracts the blood vessel will become more narrow and decrease blood flow through the facial skin. The constriction of these vessels will cause the skin to be less red. When the muscle relaxes, the blood vessel dilates or becomes wider which will increase blood flow through the skin. The dilation of facial blood vessels will cause the skin to be more red.

The endothelial cells line the inside wall of blood vessels and release potent dilator substances. When these dilators are released they diffuse to the smooth muscles cells and cause the vascular muscle to relax or dilate. When the vessel dilates, blood flow to the skin increases causing flushing and redness.

Function and Structure of Rosacea Blood Vessels

Rosacea is primarily a facial vascular disorder in which blood vessels are functionally and structurally abnormal.

Functional Changes

With rosacea there are functional changes that take place in vessels. The blood vessels in facial skin become hyper-responsive to internal and external stimuli. Vessels may dilate to a substance that normal blood vessels would not respond to. They may also dilate or open more widely than normal blood vessels, and stay open for an abnormally long time.

Structurel Changes

The structural changes of facial blood vessels, lead to the progression of rosacea.
– Blood vessels may become permantly dilated, this is known as telangiectasia or broken blood vessels
– The muscular layer of the vessels may be thin and damaged.
– The (endothelial cells) inner layer of vessels may be damaged and dysfunctional
– There may be an abnormal growth of new vessels
– Blood vessels may be closer to the surface of the skin
– Vessels may fuse together forming abnormally large vessels that can carry large vollumes of blood to the surface of the skin

Triggers

Heat

Heat will cause blood vessels to dilate, allowing more blood to flow to the surface of the skin and causing facial flushing. Heat from both external and internal factors will cause facial flushing. In rosacea skin this reaction is exaggerated.

Cold

Although cold temperatures initially cause blood vessels to constrict, over an extended time the skin cells can become deprived from oxygen and nutrients, which can trigger the release of potent dilators. Flushing caused by cold temperatures are influenced by the how cold and for how long skin is exposed to the cold factor. For example cool air will cause less constriction than cold air and would not deprive skin of oxygen and nutrients in the same way cold air does. Cool air will not cause flushing, but cold air may.

Nerves

Facial nerves play a major role in rosacea flushing. Many medical experts believe that rosacea is a neuro-vascular disorder, meaning that the facial nerves release dilators onto blood vessels which intern causes flushing.

Sympathetic nerves help regulate thermoregulatory flushing, this type of flushing helps reduce internal body temperature and serves to protect the brain from heat. If the internal body temperature is to high, the brain activates sympathetic nerves, which release neurotransmitter substances on to blood vessels, telling them to dilate and causing flushing.
Consumeing hot beverages and food, as well as exercise can activate sympathetic nerves. Even emotional stress, anger and embarrassment can trigger these nerves.

Parasympathetic nerves are partially responsible for flushing caused by certain foods, crying, facial movement, and even facial pain. It is important to note that even mild foods can cause flushing, and each person may find they respond to different triggers.

Sensory nerves release potent vascular dilators and are responsible for flushing caused by skin irritations, and environmental factors such as wind, heat and sun.

UV rays

can penetrate the skin and activate sensory nerves, as well as creating heat in the skin causing facial flushing.

Free Radicals

Clinical studies suggest that blood vessels are often the first structure encountered by free radicals and can be the hardest hit. Causing structural damage including weakening the vascular walls. In addition to the vascular damage, many free radicals are potent blood vessel dilators.

IPL
Intense Pulsed Light (IPL) is used in photo-rejuvenation for the skin and hair removal treatments. Broad Band Light (BBL) is essentially the same thing as IPL and is used in the same way. Intense Pulsed Light (IPL) is not actually a laser. IPL emits a non coherent, broad spectrum light, rather than a monochromatic single wavelength like a laser. Basically lasers have one wavelength ( for example: 800nm) with a very specific target, and IPL uses a wide spectrum of wavelengths at the same time (for example: 500nm — 1200nm) that can target any chromophore in that range. You can think of the laser like a sniper that can precisely hit a single target with one shot, and IPL is like a grenade that hits several targets at once.
IPL

IPL uses special filters that block unwanted wavelengths. The filters can be changed to “cut off” the shorter wavelengths. The appropriate filter will depend on the depth of the intended target.

The cut off for the wavelength at the deeper end is predetermined, but can vary with each devise.

For example: Lumenis One has a wavelength of 515 – 1200nm,
and has 7 filters: 515, 560, 590,615, 640,695, and 755.
The 515 filter would block out wavelengths above and below 515nm – 1200nm, while the 560 filter would block out wavelengths above and below 560 -1200nm. The deeper wavelength, in this case 1200nm remains the same, while the “cut-off” filter can be changed to block out specific shorter wavelengths.

SELECTIVE TARGET

IPL is well absorbed by chromophores that have color such as melanin, hemoglobin. The concentration of a specific chromophore peaks at different depths in the skin. The filter used in treatment is decided based on the depth of a desired target and the color of skin. A deeper cut off filter would be used for hair removal, while a shallow filter would be used when treating vascular issues such as rosacea. It is important to remember that IPL targets color in the skin, so tanning must be avoided before treatment. Patients with darker skin should seek treatment from someone experienced in treating their skin type, to avoid complications. IPL treatments can provide beautiful results, but can also cause serious burns. It is the patients responsibility to choose an experienced professional, be honest about sun exposure and follow all pre and post treatment instructions.

The intensity of a fragrance is determined by the amount of the actual perfume oils. A perfume with a higher concentration of perfume oils will have a longer lasting and more pure scent.

Take Notes

Understanding Fragrance Composition
A fragrance generally has three notes or levels, with each scent evolving into the next. The notes begin with the lightest and evolve into the strongest. Often the strongest note or the base note starts off unpleasant, however it improves with time. The middle note helps to cover the base note untill it has “matured”. Some fragrances such as a splash may only have a top and or middle note.

Top Note / Head Note
Fades in about one minute / Lightest scent (fruity, citrus)
This is the initial impression, it sets the stage for the middle and base notes.

Middle Note / Heart Note
Fades within 30 – 45 minutes / (florals, spices)
This is the body of the fragrance with a discernible aroma. The middle note acts as a bridge between top note and base note.

Base Note/Bottom
Fades in 3-5 hours / Heaviest scent (Musks, woody notes)
This represents the true personality of the fragrance, and has the most stability and staying power. The lasting power depends on strength of the fragrance.

Application

Proper use and application for a full body fragrance experience.
Layer your fragrance to assure depth and character of a fragrance on your skin. Layering refers to applying different forms of the same fragrance family, i.e., body wash, body splash, lotion and perfume. When you use a variety of body products and fragrances with different scents, they may not mix well together, creating an unflattering result. Ideally, you would layer a fragrances from the same brand/scent, however if you know the notes that compose your fragrance, you can better choose complimentary body products.

Fragrances rise so they should be applied from the ankles up. Apply perfume to the pulse points — ankles, behind the ears, back of the knees, bosom, inside the wrist and inside the elbows — to extend wear. Don’t forget less is more.

Shopping for a fragrance

When you are shopping for a fragrance, let the person at the counter help you. They are around fragrances all day and generally know them well. Most fragrance counters will have coffee beens available. Smell the coffee beens between samples to clear the nose and increase sensitivity. It is similar to cleansing your palate with sorbet between courses. When you think you found one you like, spray a little on your wrist and wait about an hour. You want to allow the fragrance to “dry down,” so that you know how the base note will smell before you make a purchase. If you are shopping for someone else, take a list of fragrances that they like or have purchased before – this will help the associate at the fragrance counter identify the common notes, and help you choose a fragrance with similar scent qualities.

If you are like me, you probably love all the perfumed body products, like the products from Victoria’s Secret and Bath and Body Work. The fragrances are so lovely, especially when you layer the shower gel, lotion and body spray. Unfourtunatly, if you have dry skin, you may need to trade your favorite fragranced shower gel for a body wash better suited for dry skin.

When you are looking for a body wash for dry skin, look at the ingredients listed on the back label. Ingredients are listed from the greatest to least amount contained in the product. If the first two ingredients are water and sodium laurel sulfate, then that is what makes up most of your product. Sulfates are used in many cleansers and shampoos, they are detergents and help products to foam up. Unfortunately, they can dry the skin by stripping the protective lipids from the skins surface. However, products that use sulfates in combination with conditioning agents and emollients will help to prevent the skin from becoming too stripped. If you have dry skin, choose a body wash that has an emollient or moisturizer as the second and/or third ingredient. Look for ingredients like petrolatum, cocoa butter, jojoba butter, and natural oils. Most Cleansers will have some sulfates, the trick is to find a shower gel that has sulfates lower on the list of ingrediants. There are products that have sulfates as the third or fourth ingrediant.

Blog sites are websites that are frequently updated. Each time an entry is published search engines are automatically notified. Blog sites are more search engine friendly and are indexed more than traditional static websites. Blogs show up around 10 times more often in organic searches than static website.

Mentioned in a blog, now what?

If you have been featured in a blog post, ask the blogger to included a link to your website, social networking sites and your own blog. Be sure to promote yourself to maximize the benefits to you or your business. Share your featured post on your social networking sites, via email and word of mouth. If your story is popular, the blogger will be more likely to write about you in the future. Each time a blog post is retweeted, shared on a facebook wall, or shared on any bloging or social networking site it becomes more recognized with search engines. Sharing the actual link is even more powerful with search engines. Blog posts linger on the internet for a long time, and continue to be viewed years after being published. Even if you were mentioned in a blog 6 months ago, it’s not to late to promote yourself.

Follow the blog

A blog site is an active online community, where people get involved. Readers can comment and ask questions, leading to online conversations and networking. When readers are involved they feel connected. If you have been featured on a blog, it is a good idea to follow that blog and become part of that community. Following the blog also shows your appreciation and helps maintain a relationship with the blogger. Having a good relationship with a blogger gives you the opportunity to have your business promoted on the site in the future. Many blogs have social networking pages, following the blog on a site like facebook or twitter is the easiest way to stay involved. Friends and family can also get involved, they can comment on your story, adding credibility and interest.

There are many people that have embraced their freckles and love them, while others would prefer spotless skin.

Freckles are clusters of concentrated melanin, ranging in color from red, tan and brown. Some people have a genetic tendency to develop freckles, however sun exposure is the main cause of freckles. Freckles generally appear on sun-exposed areas, and will appear darker or more prominent after sun exposure. Ephelides is a freckle which is flat, light brown or red, and fades when protected from UV light. Ephelides are more common in those with light complexions. Liver spots (also known as sun spots and Lentigines) are freckles that develop after years of sun exposure, and are more prominent with age.

If You Want To Remove Freckles

The best way to remove freckles is to have a series of photofacial treatments, combined with bleaching creams containing 4% hydroquinone and tretinoin (Retin-A). A good UVA/UVB sunblock is also recommended, because UV exposure will make freckles more prominent.

Photafacial (Fotofacial) uses IPL (Intense Pulse Light) to treat skin discoloration, redness and broken capillaries. There generally isn’t any down time associated with IPL treatments, however pigmented spots will appear darker temporarily. After a photofacial any freckles or pigmented lesions will darken up and flake off. It can take up to two weeks for spots to flake off. The number of treatments needed will vary depending on the skin condition.

If You Like Your Freckles

If you are not concerned with removing freckles you can use lightening and brightening products. These products brighten the skin without removing freckles. Freckles will fade slightly, but will not go away.

If you love your freckles, you may want to skip the Therapy Night Cream from this system. The Therapy Night cream has a 4% hydroquinone, which will further fade freckles and pigmentation. The Clarifying Serum also has a 4% hydroquinone, however this C- serum is only used in the morning and is not enough to eliminate freckles. The Clarifying Serum is great for anyone that wants brighten their skin tone, and still keep their freckles.

Prevent Freckles

To prevent freckles, you need to use a good sunblock. I recommend a UVA/ UVB sunblock with an SPF 30 or higher. I also recommend sunblocks with Zinc, because it is a broad band physical block. SPF only measure UVB protection, and even if a product is labeled to have UVA protection, it may not protect from the full UVA spectrum. Topical Vitamin C ( L- ascorbic acid) can also help prevent sun damage because it neutralizes UV radiation.
